Question about my son's strange symptom?

My 12 year old son has been diagnosed with ADD & ODD but I feel it is something more than this. He drags his feet, toes are pointed towards each other (he looks like somebody that had a stroke). He has severe facial tics and digs his elbows into his ribs, he is completely anti-social, has no friends, won’t try to make friends either. I signed him up for karate thinking it would help with his self esteem but he refuses to talk to anybody in the cl but says he loves karate (he has been going since he was five years old!) He goes around scowling at people all the time He has no sense of personal hygiene or why it’s important. His only interests are video games (star wars) and his cat. He loves to torment his sister. I am always correcting him on the ways he speaks but it doesn’t seem to sink in. He gets good grades (I have a reward system that seems to enourage good grades). Could this be something other than ODD & ADD? He is taking zoloft, guanfacine and risperdal. He is completely unbearable to live with without medication and causes havoc in home. I am very worried about him as he seems so isolated. He is also very clumsy.
